Most
patients are referred to our office by their general dentists, but you
do not need a referral to come here. At your first appointment you
will fill out a medical history and provide information for insurance
and billing. An examination can take up to an hour and may include
X-rays. If you are having pain, that
problem will be addressed first.
In most cases a periodontal probing chart is prepared (you probably have had this done at your general dentist) where the pocket depths are measured and recorded at six or seven places around each tooth. The probing chart, in combination with radiographs and your medical and dental history help us create a treatment plan that fits the treatment objectives of you and your general dentist. Before beginning treatment we communicate with your dental insurance carrier in order that you receive the benefits that are due you. We maintain communication with your general dentist throughout your treatment and during maintenance visits in the future in order that your dental care remains coordinated. Click here if you would like to download your medical history forms to print out so you can fill them out at home. Please do not email them back to us, but bring them to your first appointment. Our office is fully compliant with HIPAA federal requirements. You may read about the privacy rules we abide by here. Click
here if you have been referred to
us as a patient.
